Obtaining an Application Form There are two ways to get an application form: A. Download it here: SD Application Form 2008 (size: 328k) (includes Teacher Recommendation Form) In order to read and print out the above document, you will need Adobe Acrobat Reader. If you don't have it, download it from Adobe's website. B. Send for a catalog: Send ATDP a self-addressed 9"x12" manila envelope with $1.48 in postage: UC Berkeley ATDP Graduate School of Education 3639 Tolman Hall Berkeley, CA 94720-1670 Indicate "SD" (Secondary Division) on the lower left-hand corner of the large, 9"x12" self-addressed return envelope.
